Using DD/DS 3.5 disks and dskini I have failures on all of track 12. Why?

Have you tried SamDisk?, as I suppose that you're using the right image and writing over good floppies, it must be a dskini issue. Check Samdisk, it's probably the swiss knife of floppy tools
