Mystery controller card

I have here a controller card that needs to be identified. It's ISA, has two 34-pin connectors and two 20-pin connectors. The model number is NDC5425 and then printed 05AA right next to it. So NDC542505AA. Looks like an MFM or somesuch. I'll get pics soon.
