SCSi to Pertec interface

Is the 4th switch bank that is on the bottom of the board shown in that manual?
 
My board has 4 switch banks, 1 on the left side, 1 on the right and the 2 at the bottom.
The scanned document is most likely from a version before this board.
 
Mine is exactly like the picture earlier in the thread, mounted standing up so the 2 switch banks and the LED bank are on the left and one switch bank on top and one on the bottom.
 
Hello, I'm working on my Qualstar 1054 SCSI, and I'm stuck on the card setup and diagnostics. I haven't found the manual addendum mentioned here anywhere! Would you know where I can download this manual?
