It is missing the front plate that would cover everything except the L.E.D.'s.

UPDATE: I have found that I have a second one of these except this one is Rev. 3. The outside appearance of both units is identical, right down to the missing front cover plate.
I have also found an original Shiva power adaptor p/n PS-650-0101 (9VAC 1.5A) - Both units power up.
