You may also want to try reprogramming the CPLD (it's a JTAG device, so no big problem).

So a quick update. My friend is looking over all the solder joints with a magnifying glass today. I also sent the latest bios from lo-tech to him to flash. I am waiting to hear back if the CPLD is an -xl part or not. However, the part was built very recently so unless the life span of the chips is in hours we should be okay on that front. Thanks for all the info/help so far.

All right, unfortunately nothing good to report. My friend flashed the BIOS from Pierce and the card starts up. Gives the menu but the HDD is still not recognized (which was being recognized previously). Tried both the XTIDE REV1 and Chuck mod to no avail. The CPLD is an XL part (Xilinx 9572XL). We have checked the solder joints and nothing stands out. He is going to go over them again just to be double sure. Any other ideas?

AFAIK Atmel ATF1502AS/ATF1504AS/ATF1508AS are 5v CPLD and still in production.
They are pin-compatible with 5v Altera MAX 7000S series (EOS) but require different programmer.
